Fees

If required forms are not completed at the time of your office visit, there is a $20 fee.

Medical Records Requests will be completed for $20. A per copy fee may apply. (please allow 3 to 5 days).

Prepare for Flu season! Flu shots are just $25.

Do you like the convenience of mail order prescriptions? Complete the mail order prescription form process for only $20.

If you lose your prescription, there is a $10 Lost Prescription fee.
